  • D5: D5 is a five-year initiative to advance diversity, equity and inclusion in philanthropy. As a Program Coordination Consultant, Meghan has the privilege of implementing projects designed to build the field of philanthropy. More info at:
  • Your Way Home Montgomery County: A centralized intake and referral system for families and individuals who are homeless or at-risk for homelessness in Montgomery County accessible via a toll-free telephone hotline. A collaboration between the Department of Housing and Community Development, the North Penn Community Health Foundation, the North Penn United Way, and the Montgomery County Foundation, the Homeless Prevention Center is designed to: 1) refer consumers to the most appropriate housing or homeless service provider quickly and efficiently, 2) reduce the burden of intake and referral on the county’s housing providers, and 3) generate housing data necessary to make informed decisions about future programming and funding opportunities in the county’s housing continuum of care. The Department of Housing and Community Development has funded the Public Health Management Corporation (PHMC) to coordinate and manage the Homeless Prevention Center’s toll-free consumer hotline and intake database in a coordinated effort with its lead role in the county’s HPRP system. Capacity for Change facilitated the partnership’s start-up, visioning and strategic planning phases and is currently responsible for ongoing planning and evaluation efforts.
